We're having a bit of a debate regarding blood transfusions during an inpatient stay. One of our coders has been adding the PCS code for the blood transfusion, plus CPT code 36430 with revenue code 0391, both of which fall the the facility claim. She states she adds the PCS for the facility, and the CPT for the professional aspect of the blood transfusion.
Issue #1; I cannot find guidance indicating what is required for the "professional" component of a blood transfusion. Our providers are not writing a report or documenting their supervision in any way, outside of simply ordering the transfusion. They do not have hands on the patient, helping to administer the blood products. Can we even technically bill for the "professional component of the blood transfusion" as she states?
Issue #2; To charge for the professional component of a procedure, the CPT code would require a professional revenue code, something in the 098x category, as I understand it. And again, as I understand it, 36430 cannot be billed with any other rev code besides the 0391, is this correct?
