Can someone help me with this.

Diagnosis: "discogenic pain"
Procedure: Intradiscal injection bupivacaine and dextrose L2-3, L5-S1 paramedian approach

"disc at L2-3 and L5-S1 was accessed using a left paramedian approach in the center of the disc. mixture of bupivacaine and dextrose was injected at each disc." (with flouro)

Would this be 62311, 64475 or 62290? Thanks so much for any help with this.
