It depends on the documentation. As described, I would think the CT was part of the procedure as guidance, not a separate encounter/procedure.

There are codes for CT guidance (77012-13) that should be used for this instead of diagnostic CT procedure codes.

My thoughts...

Since follow up CT is performed after US Guided biopsy, we can bill the service separately as entirely a different modality study is done. Also it would be a very limited study of that concentrated area and hence you can use 76380 - Limited CT study
