Wiki Medicaid Dx for IUD/Implant insertion and removal

Does anybody know what diagnosis codes Medicaid accepts for IUD insertion/removal and Nexplanon insertion/removal? I'm using Z30.430 and Z30.432 for the IUD and Z30.017 and Z30.46 for the Nexplanon insertion/removal, but these are being denied every time for "Dx inconsistent with procedure". These are supposed to be covered services under the family planning policy. I've tried diagnosis codes for initial prescription and these deny as well. Help!

You are using the correct codes if the reason for the IUD or Nexplannon is contraception, perhaps your Medicaid coverage is limited to reasons other than contraception, this is typical and you should check to see.
