How do we get a final denial from Medicare for non covered Medicare services to forward to a secondary who will pay non covered Medicare charges?

Did you submit the original charge to Medicare with GY modifier to get service denied?

Can we bill this directly to Medicare? We are using this when billing to secondary that don't require a eob from Medicare.

In order to get a denial from Medicare, you must submit a bill.

How about if the code is not accepted by Medicare - for dorsal column stimulators they are now not covering the leads (L8680). Medicare will not even accept the codes. However, other insurance do and reimburse.
