Wiki Medicare/Medicaid Diagnosis Codes

If a physician does not provide a diagnosis code on a requisition for a government payer (ie: Medicare/Medicaid), is it OK to obtain a verbal diagnosis from the physician's office?

I work for a pathology lab and we have to call medical offices all the time for diagnoses. I would suggest patient chart note reflect date, who at the office gave you the diagnosis and if so inclined get them to fax you something.
