I received the following from Medicare on the CPT codes below. Has anyone seen these requirements for these ultrasound codes before? I can not locate these requirements anywhere on Medicare's website.

76376: requires a tech licensed in CRT
93975, 93976: requires tech licensed in ARDMS: RVT, OR CCI: CCT
93325: requires MD licensed in Cardiology

I don't know where you're located but NGS J13 has an Independent diagnostic Testing Facility Matrix on their website under their Resources/Tools and Materials section. This list the requirements for diagnotistic testing and who can do them. I'm sure other MACs have similar information.
