When appending -25 for multiple e/m visits, do i appended -25 to both visits or just one? Example 99381 -25 99221 -25 or 99381 -25 99221. This is a big dispute in our office, i learned that -25 is appenped to seperate identifiable procedure or services only, can somone give clarity, please
 
Thank you your quick response. My question is do you append -25 to both encounters? Example: A patient come to clinic for a periodic comprehensive preventive medicine visit (99391) but is also seen for rash (99212), we charge for both encounters, do I append -25 to both 99391 and 99212 or just one?

You need to check your new codes for ICD-10 CM .. If the patient presents for a routine preventive and has a symptomatic complaint then cannot perform the preventive, you must reschedule it. Look at your Z00 codes. The category states encounter for general exam without complaint, suspected, or reported diagnosis, and the excludes 1 note indicates that encounters for signs and symptoms cannot be coded here.
The CPT instruct for the use of an office visit with a preventive indicates the same thing. If the patient presents asymptomatic and the provider "discovers" an abnormality then you may chose the diagnosis code indicating abnormal finding, then you may bill the preventive code and the office visit level. The 25 modifier is appended to the office level.
