I have a staged procedure. 1st CPT codes used were 11012-F8, 11760-F8 and 12001-F8.
3 days later CPT codes were 26735-F8, 26727-59-F8, 15736-F8 and 11012-F8. Where should 58 modifier be placed since CPT codes are not same in 2nd Procedure? Thank You! Welzi.

I would put the 58 mod first since that will release the $$ at 100% rather than as a complication (78 mod) reimbursement..then add the other mods (59, F8, etc.).
