A patient is having a manipulation under anesthesia for arthrofibrosis during the post operative period for a total knee arthroplasty. The patient was informed prior to the initial TKA surgery that they may need to have the manipulation done post op.

Is the appropriate modifier for the manipulation a 58 due to it being anticipated or a 78 due to it being a complication?
 
Definitely use modifier 58 as this is an anticipated issue with the joint replacement, and the physician documented that he "planned" on doing it post op if it was necessary
78 is a return to the OR during a post op period, usually for an infection or wound dehiscence
