Should we be using modifier XS when an outpatient is receiving--for example--and IVP and also IM/SQ injection(s)? Should it be used on the injection(s) and each injection on a separate line?
 
If you have a tool that you can run your codes through for the CCI edits, it will direct you to apply the modifier if applicable to the column 2 code and give you that code. In this case it would be the injection code.
