Who should report modifiers 80 or 81 at time of claim submission? the Assistant Physician or the main performing physician?

The assistant at surgery or assistant surgeon claim is the claim you append the 80/81 to.
Think about it - you wouldn't append them to the primary surgeon's claim because that would be like telling the payer that the primary surgeon was the assistant.

Also note there are some payers that don't want the 81 for physician assistant, nurse practitioner, or clinical nurse specialist, they want AS.
