I initially billed 99214-25, 94760-51,94370-51,94640-51,94370-76. The only thing that was paid was the E/M, the rest of the cpts were not. denial reason was modifier not appropriate for procedure submitted. What mod am I supposed to use if any? Help!

No modifier is needed on any of the above codes except the 99214-25.:)
