Please help, does anyone know how to code the following:

1)Fluoroscopically-guided, lumbar hardware block at the right L4 pedicle screw.
2)Fluoroscopically-guided, lumbar hardware block at the left L4 pedicle screw.
3)Fluoroscopically-guided, lumbar hardware block at the left L5 pedicle screw.
4)Fluoroscopically-guided, lumbar hardware block at the right S1 pedicle screw.
5)Fluoroscopically-guided, lumbar hardware block at the left S1 pedicle screw.
