I donot have a HCPCS Book and need to know what is the codes for

1) Ceftriaxone 500mg IM

2) Cofactor (I think this is right) 250mg IM

I can't find the Cofactor let me know if it is something different, but the other one is

J0696 Injection, ceftriaxone sodium, per 250 mg, So it looks like for 250mg you would do x2
