I am really in need of some general assistance with diagnosis coding for Home Health. I am a CPC and work for a Home Health Agency doing their billing, and they have asked me to start coding on the Oasis SOC, but I have general questions...
1. Is it accurate to say that you can only code from the problem list/diagnosis list provided by the referring MD source? Field nurses are not permitted to choose the diagnoses unless they are on the medical record from the MD, correct?
2. Do you list the diagnosis codes on the Oasis and THEN the admitting RN does the Oasis/picks the relevant diagnoses, or do you review and correct the diagnoses after the RN has selected them?
3. How do you handle adding diagnoses for recert episodes if the reasons for care have changed since the SOC?
