A patient comes into a neurology clinic after testing done for symptoms of HA and vision complaints. At f/u visit pt has no complaints and no symptoms, doctor discharges pt with only survailance. What icd-9 code would you use if pt is no longer having ha, no is the doctor treating them with anything?
 
I would have to strongly disagree w/ using dx that aren't there. I would go w/ V12.49 personal history of disorders of the nervous system and sense organs.

There are follow up V codes that could be used in this situation. I don't have my I-9 book on hand at home, something like follow up exam?
