Question: Has anyone encountered a request of consultation from a parent with non face to face with the patient? Would there be an E/M for that--if so, what would it be?

Non Face-to-Face consultaiton with a guardian

I have had similar scenarios in oncology from time to time.
The primary diagnosis code in this instance is V65.19,Other person consulting on behalf of another person, and most likely this won't get paid. Medicare must be face-to-face. I think you will have to consider this good provider-patient relations and a no-charge visit.
