We have a nurse practioner working with a doctor at one of our clinics and wants to see patients on friday's but the doctor wants to take off on fridays, can she still see those patients. My understanding is the doctor has to be house to bill out charges for np. The np does not have her own mcr number, all her charges are billed incedint to under the doctor at the 85%. Now if she had her own mcr number we could let her see patients by herself or am i wrong? Thank you for your time.

Strictly from a Medicare perspective, the incident to rules require direct supervision. Per the Medicare Benefit Policy Manual 100-02, chapter 15, section 60.1 (B):

Direct supervision in the office setting does not mean that the physician must be present in the same room with his or her aide. However, the physician must be present in the office suite and immediately available to provide assistance and direction throughout the time the aide is performing services.
