I'm coding in an ASC and the diagnosis on the op report is Headache secondary to greater occipital neuralgia versus cervical facet mediated. Since the doctor has not decided what is causing the headache should this be coded 784.0 and the cervical neuralgia & cervical spondylosis? Input would be appreciated.
