I need advice on accurate coding for this scenario. A doc in our practice started a LHC. He got as far as revealing the LV function then stops the procedure and leaves the sheath in place. Another doc (in our practice) comes in later that day and finishes the cath and gives the angiography results (everything but the LV) I think i should code for doc A: 93510-26 then finish up with doc 2: 93543-93545-93555-26,93556-26 but this isn't going to go over well with MCR. what would yall do?

I just took my CPC exam, but I'll give it a shot...

One question...why did the first doctor stop?

On the surface, I'd say MCR is balking on doc 1's -26 modifier. Also, looks like the cath should be a one-doc procedure.

With that in mind, I might think this way:

Doc 1: 93510-53 (discontinued procedure)
Doc 2: 93510-77 (repeat procedure by different physician), 93543, 93545, 93555-26, 93556-26

Like I said, just a shot, but if someone has a better solution, I could use the education...

this is an unusual scenerio....I would ask Dr. 1 as to why he did not do the full procedure....if he was planning to do only the cath and Lv then code what he did. If he performed the LV or left ventricle angio then code 93510-26, 93543, 93555-26

for Dr. 2... I would code as follows:

93508-2659 - cath placement
93545-59 - coronary angio
93556-2659 - coronary angio S&I

I hope this helps,

Dolores, CPC CCC
