Hi Group, I am relatively new to Gastro- I work for an ASC- we have just started doing Colonoscopies and hemorrhoid procedures- My question is: where can I get examples of what is needed for documentation for procedures: for example: 46221 Hemorrhoidectomy by simple ligature- what should the OP report state-Or for Hemorrhoidopexy 46947-Are there key descriptors that must be on the Operative report? I appreciate any and all help.

I think that it is really important for you to have the Coder's Desk Reference or similar product that gives you a written description of the most common way a procedure is performed. This will give you insight on what to look for in your operative notes.
