Wiki Optometry Billing - Adding Material Copay to same line item with original copay

I encountered a claim where the material copay of 25.00 was added to a covered item that already had a copay of 55.00 totaling to 80.00 copay.
If the covered item has a copay assigned to that item should a material copay be added if this is the only covered item or should the material copay be removed or not added?
Any help or leads to credible sources would be great,
