If our orthopedic surgeon sees a patient in the office for consultation for a fracture (for example...S82.61XA). I would use the 7th character 'A' for the initial encounter. What would I use for a follow up visit after a CT scan? And what would I use for the surgery? I understand 'A' is only for the initial encounter but the next choice would be 'D' for subsequent encounter for a closed fracture with 'routine healing'. Since that fracture will not start healing until after surgery, what would the most appropriate 7th character to use for visits after the initial leading to surgery?
And after surgery is done, we won't know if the fracture is healing routinely or delayed or with malunion until the post op visit xrays are reviewed by the surgeon. HELP! :) Thank you.
 
It will remain the A until active treatment is completed. In this case that will be the surgery. The it will go to routine healing until the provide indicates different.
