An established patient was seen to request a referral for weight loss surgery. A physical exam and ROS were done, but no other issues were discussed, just the referral for weight loss surgery. What diagnosis code should I use for this? Is this even billable?
 
Did the provider document any counseling and/ or screenings done for this? I usually end up coding diet/ exercise counseling with the obesity code and bmi. Or diabetes screening If none of that was done, I wouldn't think it could be billed.

Seems like one of those things you'd charge the patient upfront for.
