I am somewhat new to pathology coding and we bill the TC portion of the pathology. I am curious about adding diagnosis codes when the report comes back with MLH1, MSH2 , MSH6, PMS2 being positive. Would you add the Z15.09 to the claim? There is a finding of rectal cancer in this case so I would enter that as the primary diagnosis code.
 
No, please do not add Z15.09 to the rectal cancer. We perform several TC only charges where I work. When billing TC charges no interpretation was performed (and no professional fees were billed) by any pathologist we would simply bill the IHC (immunohistochemical stains) with the diagnosis code or code(s) provided on why the stains were applied.
