I need help distiguising between a screening and health risk assessment for Tobacco and Alcohol. We have some providers wanting to bill 96160 due to asking the questions during social history with patient. There is no screening too/instrument being utilized.

This is all we are locating in chart note below, is this enough support CPT 96160?

I say yes that is enough support CPT 96160. However if pt is a smoker can use CPT 99406-99409 series but add minutes in record with statement to discourage pt from smoking counseling for so many minutes listed in documentation . Use the dx F17 not dx Z72 if current smoker linked with CPT 99406-99409.This can be used with Eval Mgnt CPT codes 99202-99214 and add modifier 25.
