Can you use negatives in the past, family or social history?

Example:

Past history - none
Family history - none
Social history - Drinks alcohol every day and smokes 2 packs a day.

Would this still count as "3" or "1"?

That is going to be a carrier descretion... most carriers will say no it's not a complete pfsh but I have dealt with some who will accept that, but if you really think about it, family history may not be pertinent based on the presenting problem the pt is having.

With alot of the pt's we see family history is usually relevant especially in the elderly.

Or if the pt is not medically able to give the information it would need to be documented why the physician could not get the info, say the pt is comatose or someother ailment, then it's possible but just saying "none" would not cut it.

Anyone else have any thoughts on this.
 
it counts as 1
there has to be a statement as to what was reviewed....all systems negative, none and negative are not sufficient...remember, allergies and LMP can count as past hx in case they were mentioned...these are sometimes overlooked and only counted as ROS...good luck
