My doctor was called by a doctor in another state to consult on a pt. My dr has never seen this pt. The dr in the other state told my dr that he should be able to bill for this phone call. What do you think?

Unlisted E/M 99499 maybe?

I don't see anything else that would work.

99441-99443 have to be established patients, and it is provider to patient not to another provider.

99366-99368 again have to be established and there must be a minimum of 3 different speicalties involved.

Just my thoughts,

I think you will need to do the unlisted.. send along the records.. and see what happens. Are you billing insurance? Since the patient is out of state, does your doctor even participate with their insurance? If you do participate, and they don't pay for non-covered, you can of course bill the patient. If you're non par and they don't pay, I'd bill the patient no matter what.
