Good morning. This may be a simple question - for the diagnosis code(s) on the referral from the physician's office, do those codes HAVE TO BE on the patients claims? We have a patient who has a couple of diagnosis codes related to the knee from the physician's office. The physical therapist coded it as a different diagnosis code (still the knee).
