I need a little direction on this. I'm not all that experienced with EP billing and coding and am not having much success finding what I need, unless it's right in front of my face and don't realize it.

We billed:
93651
93620-26
93621-26
93623-26
93613

United Health Care paid the 93651 and denied the rest stating that they were not covered and their decision was based on an NCD. I can't find one with CMS. I've even called and their 'representatives' say they can't help. Maybe one of you can??
 
We had problems with United Healthcare not paying 93651 when billed the same day as the EP study. Add modifier 59 to 93651, the dx should be atrial flutter.
