We are having difficulty with pre-op consults and need help! Here's the scenario:

Pt seen by PCP for knee pain-Pt has known HTN and Diabetes. Pt is referred to orthopod for knee pain. Orthopod recommends TKR and sends Pt. back to PCP for pre-op clearance. Should we bill pre-op or Office E/M since PCP has knowledge/History of HTN and Diabetes.

The PCP may code a consult based on the fact that the orthopod is requesting his/her opinion for surgical clearance. The consultation requirements must be met (Request/Render/Report back to surgeon). Dx coding would be V72.8X, reason for surgery, HTN, DM.
