We have a difference of opinion on whether or not to use medical diagnoses for a preventative exam CPT?
If there is no support for a separate E/M level to be billed, should the diagnoses of anything else be listed?
 
You can list a medical diagnosis on a preventive exam code, just not as primary. If the Dr. sees the pt for a medical condition in addtion, if their documentation supports a separate E/M, you can bill that as well with a 25 modifier. Many payers do not reimburse for this separately though. I tell my physicians to have the pt come back to the office for a separate visit in this scenario.
For Medicare IPPE's and AWV's, you can list any diagnosis code on the charge for G0402 or G0438 or G0439 per MEdicare.
