When a pt come in for a PPD vaccine and have vitals done, can we charge a E/M Code? Sometimes it is done by NP or MA but rarely by MD but MD does sign office note.

The E/M would have to stand alone, if all you have is vitals and a signature there is not enough to support an E/M.

If the reason they came in was for the PPD that is all you should be charging for unless there truly is a significant and separately identifiable E/M service provided.
