DOS 11/2/09 --If a IM Resident (under permission of Admitting Doc) admits a patient @11pm and did a H & P, can the Admitting Doc charge for the H & P on that day and then charge for establish visit on DOS 11/3/09?
Government payers (Medicare, Medicaid, AFDC) require face-to-face contact by the teaching physician. So you would code the Initial Hospital Visit on the date the TP FIRST saw the patient (11-03-09 in your scenario).
Commercial payers typically allow the "supervision" to be done by any means, including a phone call. So if the resident completed the evaluation and admitted the patient on 11-02-09 after speaking with the attending on call by phone, then you could code the Initial Hospital visit on 11-02-09.
