History of colonic polyps, new colon polyp found. Would you code it as 211.3 and V12.72? or drop V12.72 and just use 211.3............

You would use both, under the guidlines it does state A history of an illness ,even if no longer present ,"is important information that may alter the type of treatment ordered."

You would use both, with 211.3 as the primary diagnosis and V12.72 as the secondary.

well thank you both! I have people telling me to drop it and people telling me to use it. I feel more comfortable using the history code. So I will stick with that. Thank you both again.

If it is a Medicare patient, V12.72 would be the primary/principal DX followed by 211.3.
