I am new to CHONC world and don't have someone I'm training under, can anyone answer a question I have: If you are coding for an encounter for radiation therapy and they have a secondary malignancy that they are treating, do you still code the primary malignancy too? I was told that we don't need to, but the person that told me is not a coder. Any help is appreciated!

Hi,
Regarding encounters for chemo, immunotherapy or radiation if treatment is being directed to the secondary neoplasm then it should be designated as the primary diagnosis followed by the primary site. You may also need a Z code (Z51.11, Z51.12, Z51.0) for the encounter, most payers want it listed first followed by the malignancy sequencing. I would definitely check with your payers first about the sequencing of the Z code as I'm in Ohio & our MAC carrier is particular about how the Z codes are listed on the claim. Most of the other payers want it listed first. Hope this helpful!
