What diagnosis code should be reported by the outpatient hospital radiology department and the interpreting radiologist when the reason for the exam was ruled out. For instance, DXA scan ordered with reason for exam "osteoporosis" but the exam is negative (and no indication that the patient had osteoporosis in the past but with treatment has increased bone density). Or MRI knee is ordered for "medial meniscus tear" but the exam is negative and no tear is shown.
