Noridian has recently denied payment on some of our E/M codes that are billed with dx 786.50 or 414.9; invalid codes Has anyone else been having this problem?

Those are unspecified codes, perhaps the denial reads dx code lacks specificity? With the specificity of ICD-10 CM the payers do not approve of the widespread use of the unspecified codes. Many have indicated they will be denying these codes unless you can justify the usage by indicating on appeal that there was no way the provider could or should have been able to have access to more specific information.
