What DOS should be used when billing for remote device monitoring services (CPT codes 93294-93299)? Is it the first day of the monitoring period, the last day of the monitoring period, a date range to cover the entire monitoring period, or a different date? Any help (answers or links) would be greatly appreciated.

i use the date of service of when the report is downloaded. this is due to the fact that many payers will on allow the range of service dates when patient has the monitor use during a 2 month period (example -- dos beginning service 01/15/14 and ending date of service 02/14/14).
