Can anyone tell me were I can find rules governing replacing handwritten encounter sheet with the latter transcribed notes on same encounter?:confused:

Don't know exactly where to find rules/regulations regarding this, BUT...

We do not "replace" the handwritten notes. We add the dictation. Both remain in the patient chart.

This is what I have always understood. But the office managers at my current job say this happens all the time. One doctor even said Medicare requires it! So having this in authoritative print would be of great help in educating them.:cool:
 
I agree with Tessa on this one. I have never heard of Medicare requiring that handwritten notes be replaced. I know that they require notes to be legible etc. but not of replacing those notes. Could you ask your office manager where that information came from? Is this maybe related to the push for EHRs?
