Wiki Requirements to bill for a Pre-op Consult

If a patient brings a History and Physical form provided by another physician to be completed for a Pre-Op exam and we put a copy in their chart, does this fulfill all of the necessary requirements to bill a 99242/99243 (depending of course on the amount of documentation in the progress note?) We use V72.83 as a diagnosis code. Thank you in advance for any feedback you can provide.
