Wiki Rhizotomy or neurotomy documentation

Hello,

So I know a rhizotomies and neurotomies are basically RFAs, so we would use cpt codes 64633-64636. However, are the the documentation requirements the same? Meaning do we need temp and duration documented in the op notes for a rhizotomy or neurotomy?
 
Hi,
I would imagine you would. Per chapter specific guidelines it states: Do not report 64633, 64634, 64635, 64636 for non-thermal facet joint denervation including chemical, low-grade thermal energy (<80 degrees Celsius), or any form of pulsed radiofrequency. To appropriately report any of these modalities, use 64999.
