I had previously asked about scar revision but I need to further clarify. The patient had an ACL revision with scar revision.

This is what the op note reads:"The previous cicatrix was excised, and then the skin incision extended proximally to facilitate harvesting the central third of the patellar tendon graft. The subcutaneous tissues were underminded on both sides of the cicatrix in order to have a later tendion-free closure."

Is this scar revision or graft harvesting?

he is using the previous incision (scar) as his approach for the harvest and at the same time excising the scar.

Since this is his approach to the planned procedure, this is incidental and not separately reportable.
