If I have a patient that comes for a screening with diagnosis code Z12.11 and there are polyps would the correct sequencing be Z12.11, D12.5 and D12.7?
 
Yes, as per guidelines:

"Should a condition be discovered during the screening then the code for the condition may be assigned as an additional diagnosis."
