I have a provider who billed for an excision of a lesion. In his op note he states " lesion was cut using scissors". I am thinking this cannot be billed as an excision for the following reasons:

1. Lesion was not excised
2. no closure was performed

He told me that I need to use the smallest measurement and bill it as an excision.

I told him I will do some research because I think the way to go is skin tag removal?
 
if you review the instructions no where does it state "scissors" as a form of excision except in skin tag removal.

no suture, no closure no excision
