Is there a cpt code for the evaluation report written for speech therapy? We have just added a speech language pathologist, and have the common cpt codes mapped out. But, they are assessment based and do not include the time taken for report writing. Is that usually a self-pay part of the eval? Please help!

The evaluation includes the report. There's no separate code for the report writing....it's inherent in the evaluation code. CPT Assistant October 2013 has an excellent description and several clinical scenarios as to what's included in Speech Evaluation codes. In all examples, there's some element of documentation (as part of the post-service activity) of a report and/or treatment plan and communication of this to the patient/family.
