The principal diagnosis is always the medical necessity (reason) for the encounter. This patient is not having the pacer removed because they have SSS, they are having it removed because of a complication (infection) due to the presence of the pacemaker. You can use SSS as a secondary diagnosis to show why they had the pacer implant initially.
