std/sti testing during annual

hey there,
which std/sti screening is included in an annual gyn if requested no symptoms. say there are extra labs done and even pap if they are not even due for pap, but pap excluded no complaints or symptoms and requesting std/sti testing because maybe a spouse cheated or for whatever reason, could you charge a separate straightforward level mdm with the annual or would that just be included?

As there is no separate code for collecting a Pap specimen (except for Medicare patients), so the collection would be included in the preventive E/M service or E/M service as part of the exam. If std/sti screening tests are ordered, you link them to the ICD10 code that indicates the reason. Simple screening is Z11.3, if the patient is worried due to sexual behavior of a partner you can add Z72.51 as an additional code.

thanks! at what point does std/sti testing billable as a separate e/m because it is not part of a routine annual or is is considered part of a routine annual if someone asks for it done? if they had symptoms of itching/burning and discomfort and then ordered it i would then think it's seperatly billable. it doesn't seem like my practice does std/sti lab tests at every annual so that doesn't seem part of an annual. would you bill seperate e/m for request of them with no symptoms?

If they come in with complaints and the complaints are addressed, then it is a problem visit, and you use MDM to select cpt. Also add on code 99459 can be added if pap smear is ordered and a chaperone is present.

(would you bill separate e/m for request of them with no symptoms?) No, its kind of a lab/nurse, if they run test and no diagnosis is found, (and don't think this service isn't being counted, the lab bills out for the test, you just cant bill for medical decision making.).
