Scenario:
A patient is prescribed 16mg of suboxone daily, to be administered in pill form BID. One 8MG pill in the morning, and another 8MG pill in the evening.
On the CMS-1500 form, which is the correct way to bill:
  1. J0574 at 2 units, using the NDC number for the 8MG pill in box 24A on one charge line
    1. If this is the correct method, after the NDC number would you amend with a ME16 for total usage for the day or would you amend with a ME8 for the individual dose?
  2. J0575 (10+mg at 1 unit- rolling total milligrams administered for the day) and using the NDC number for the 8MG pill in box 24A
